LOL I was born in middle of night and I think Dr Sussman was either sleepy (or tipsy) he got my mom's age wrong (by 3 years) also he wrote down she was a housewife, she was actually a nurse back then

Doctors then were even more loath to do paperwork than now. It was probably filled out by a nurse or clerk and the only thing that was actually from the doc was his signature.
At one of the hospitals here in the eighties if possible they would have the student nurses obtain the info, fill out the checklist form for birth data, give it to the ward clerk, who filled in her part with addresses and such and typed it out on an official form. Then put it in the docs mail box for their signature. Where it went from there and how it got the "Official Seal" I don't personally know.

mine was a copy which I had gotten from the Health Dept in Manhattan 20 years ago
I went down and waited in line and paid for the copy
it had the Seal of the Health Dept stamped on it
but I still had long conversation with the gov. bureaucrat on phone last year whether he would accept it
Bureaucracy about birth certificate is now incredibly strict!
