Reporting from Washington -- President Obama suggested at a town hall event Wednesday night that one way to shave medical costs is to stop expensive and ultimately futile procedures performed on people who are about to die and don't stand to gain from the extra care.

In a nationally televised event at the White House, Obama said families need better information so they don't unthinkingly approve "additional tests or additional drugs that the evidence shows is not necessarily going to improve care."

He added: "Maybe you're better off not having the surgery, but taking the painkiller."

Obama said he has personal familiarity with such a dilemma. His grandmother, Madelyn Dunham, was diagnosed with terminal cancer and given less than nine months to live, he said.

She fell and broke her hip, "and the question was, does she get hip replacement surgery, even though she was fragile enough they were not sure how long she would last?"

Obama's grandmother died two days before he was elected president in November. It was unclear whether she underwent the hip-replacement surgery.

The event, hosted by ABC News' Charles Gibson and Diane Sawyer, gave Obama a prime-time forum to promote his healthcare overhaul. A total of 164 guests were invited. ABC pre-screened questions, though the White House was not made aware of what they would be.

Republicans described the event as an "infomercial," faulting ABC for giving the president such valuable TV time in the midst of a high-stakes partisan policy discussion.

The audience -- which included doctors, patients, health insurers, students and people with ailing relatives -- clearly was unhappy with the current healthcare system. Gibson asked for a show of hands to see how many wanted to leave the system unchanged. No one raised a hand.

I can see his point. Sadly we have come down to those choices. I sure don't want the law or insurance company making the decision that families and patients should be making. That's where we are headed though. Just like with everything else in our lives. Heaven help us.

Edit: remember the stink that was raised down in LA or some place where an organ transplant was denied a kid who had other issues and wasn't going to live. I'd have to look up the story to get it totally correct.

The above statement is not quite correct.

The author should have stated that situation pertained to...the masses...not the elite that would be given all desired care because they could afford it.

Yes, in general our system is screwed, but buckwheat care would be so much worse I cannot tell you.

Many doctors are dropping out of 'the system,' and accept patients for a hundred bucks, or so, per month; and when they need some sort of medical attention, they pay nothing else. Except if it requires a hospital visit, or some extraordinary procedure.

The paperwork and record keeping/billing costs are off the charts with our current system.

I sure don't want the law or insurance company making the decision that families and patients should be making.

Families most likely will always opt to extend life as long as possible. A better option would be to require everyone to specify in a personal directive the circumstances under which the individual would no longer want to be on life support. Those that opt to have life prolonged under circumstances considered hopeless by a medical panel should be required to pay a portion of the medical costs from their estate. Then again, if we stopped the craziness in fighting Israel's wars and abandoned military bases abroad we'd have more than enough money for the best medical care in the world for all who need it, even those that don't, just for practice and to keep busy.

Mostly because every system has its own paperwork requirements; there is no standardization in the industry.

Many medical offices have more people to deal with the insurance companies and their insane coding requirements than they do practitioners who see patients.
