This is a stock photo from Wikipedia. It's actually called Morton's Toe. The French call it a Neanderthal toe and it's supposed to be associated with intellignence and royalty. Mine's not this ugly.
I've also heard it called a Celtic toe. If someone has one it's a very good sign they have Celtic ancestry.
Neanderthals had big brains but not much prefrontal cortex. It was all in the back in the visual area.

My Big toes are even with the third toes and NeanderToes are slightly longer than both. Mine are much more symmetrical and aesthetically pleasing than the photo. The little toe in that photo doesn't even look like it belongs on that foot.

I've never heard anyone associate intelligence with royalty, although I have many times read of royalty being associated with inbreeding.
