First and foremost, this is not authorized by federal law, specifically 5 USC § 552a. In fact, some of the questions violate the terms of the Privacy Act and the Freedom of Information Act. Using this questionnaire on a policeman could result in being arrested for obstruction of the police. City of Billings v. Skurdal (1986) 224 Mont 84, 730 P.2d 371, cert. denied 481 U.S. 1020. In any case, it won't in the slightest way prevent the policeman from ticketing and even arresting you. State v. Booher (Tenn. Crim. App. 1997) 978 S.W.2d 953.
