An herbal supplement that is widely sold in the United States and Europe to relieve urinary symptoms in men with an enlarged prostate has no benefit over a placebo, a US study said Tuesday.

The global market for saw palmetto extract is about $700 million a year, but a randomized trial at 11 sites in North America showed that even triple doses of the over-the-counter drug neither worked nor harmed the patients.

"Astonishingly enough, there was not any measurable effect -- either in benefits or in toxicity -- with increasing doses of the supplement in comparison to placebo," said co-author Claus Roehrborn, chairman of urology at University of Texas Southwestern.

"These supplements are apparently not doing anything measurably above and beyond what we call the placebo effect," said Roehrborn of the research in the latest edition of the Journal of the American Medical Association.

Led by Michael Barry of Massachusetts General Hospital in Boston, researchers followed 379 men age 45 and older whose symptoms included difficulty emptying their bladders, weak and/or frequent urination.

As part of the randomized trial, some received saw palmetto extract -- which comes from the berries of the saw palmetto dwarf plant tree -- and others were given a sugar pill that smelled and tasted the same.

Measurements showed the drug, even when increased in dosage over 72 weeks, had no impact on urinary symptoms such as nighttime urination or incontinence, and did not improve sexual function or allow men to sleep better.

"None of them showed any effect whatsoever in contrast to placebo," Dr. Roehrborn said. "These supplements cost about $30 or more a month, and they obviously don't help."

Enlarged prostate is a common condition of aging that affects about half of men in the United States over age 50, and 75 percent by age 80.

The growth of the prostate can make it difficult to urinate and can cause urinary tract infections.

Medication and surgery are some of the treatment options, though herbal supplements with anti-inflammatory properties have also been widely used for decades in the belief that they can ease symptoms.

The men in the study did tend to experience a slight improvement in symptoms, but the trend was observed in both groups.

"We commonly see this in clinical trials," said co-author Gerald Andriole, chief of urologic surgery at Washington University School of Medicine in St. Louis.

"Patients often report an improvement in symptoms because they are taking something, even if it is a placebo. But in this study, there was no benefit to taking saw palmetto over the placebo."

The research was conducted under an investigational new drug application from the Food and Drug Administration, and was funded by the National Institutes of Health and the National Center for Complementary and Alternative Medicine.

The saw palmetto pills and placebos were donated for the study by Rottapharm/Madaus in Cologne, Germany.

Comments: Deacon John I take Saw Palmetto twice a day and it has helped me greatly. Before I had to urinate every two hours at night, constantly being awakened by the urge, but now I usually sleep all night with an occasional "have to go"! Also my Saw Palmetto only costs about $12.00 for a few hundred capsules, not $30.00 as the report said. Walmart or Walgreens often offer a Buy 1, get 1 to save more money. I recommend continue taking the natural remedy. what about when it's used in combo with stinging nettle, pumpkin seed oil, zinc, beta sito sterols etc? doesn't sound like a very thorough work up. Paul Study would have been more interesting if something like pharma's Flow-max was included.

Study paid for by a pharmaceutical company perhaps??

If not then who paid for it? Typically natural remedies are never clinically researched for effectiveness because there's no profit sufficient to cover the cost with unpatentable treatments or cures.

I was getting up to whiz three times a night so I used saw palmetto and the problem went away.

I tried saw palmetto. Results were nil. I suspect that it would require some enormous dosage of saw palmetto - like a whole bottle of pills every day - to get the promised benefit.

Curative response likely depends on how the herbal was prepared and on an individual's physiological/neurological characteristics.

Flomax seems to be doctors' preference these days: Tamsulosin (Flomax, United States and Canada) More drugs used for BPH. Use:

Tamsulosin or Flomax is used for benign prostatic hypertrophy. How It Works:

Tamsulosin or Flomax is an alpha blocker meaning that it blocks a specific part of the nervous system - the alpha part. This blockage reduces the nervous system messages sent to muscles in the urinary tract specifically the bladder neck and the prostate. This results in relaxation of these areas and enables voiding. Side Effects:

Tamsulosin or Flomax is generally well tolerated and side effects are not common. However, Tamsulosin or Flomax can cause a drop in blood pressure resulting in syncope or fainting. Other reported side effects include dizziness, headache, nasal congestion, and heart palpitations. Cautions for People:

Because Tamsulosin or Flomax has the ability to affect blood pressure control, it must be used with caution by people who have low blood pressure or who are taking blood pressure medication. Older people are more at risk for blood pressure problems and should be closely followed. Not all men with benign prostatic hypertrophy or BPH are suitable candidates for the drug and care must be used to ensure that it is used in the appropriate instances. Drug Interactions:

Tamsulosin or Flomax has the ability to alter the effectiveness of blood pressure medication. -----------------------------------------------------------------------------

A non-drug/surgical alternative is an in&out urinary catheter. Plastic ones cost a buck and when sterilized in a salt solution can be used over and over again. Always a risk of bladder infection and in turn bladder stones which if not enough water is drunk (gallon daily) get so big that they can't be fished out because of the small eyelets in the catheter tip. Don't know if anyone makes a catheter with a single large inlet hole through which bladder pressure can push out larger stones. I can dump up to 600 mls with a catheter. Stones could also be due to exceeding the daily servings of fruits and veggies or dairy products.

Flomax is expensive, is there an alternative?

Yes, (see How do I bring my PSA down? Above.) Flomax is only at best a short-term fix which does not address the underlying cause which is an enlarged Prostate. If you do not deal with that, it will eventually get worse. Moreover continued use of Flomax will increase the risk of eventually becoming incontinent because Flomax is weakening the sphincter muscles by keeping them permanently relaxed. The analogy is imagine you have a blocked pipe which reduces flow. Instead of dealing with that blockage you take out the valve at the end of the pipe. that may temporarily increase flow but the blockage is going to get worse. Eventually the open valve is no longer sufficient. Ironically, if someone has been on flomax for a long time, dealing with the blockage can initially make things worse since the muscles will now be too weak to handle a stronger flow.

All drugs have side effects and reduce your health and well-being, therefore only to be taken in emergency and never long term. From my experience with BPH sufferers over the years Flomax rarely works that well anyway but has a number of side effects that are not dangerous but nonetheless are unpleasant. The most common of these include erectile dysfunction, headaches, fatigue, nasal congestion, dizziness, itchiness, and syncope - a sudden loss of consciousness.
