It is apparently available at Duke in Durham, NC. It looks like a very good alternative to a biopsy. What say you? Larry Hi Larry, MRI is not generally seen as an alternative to biopsy. I believe that the advanced prostate cancer risk assessment which forms the basis of my prostate protection program which uses TRUS (Trans-rectal ultrasound scan) is a far better alternative to biopsy and I will explain below. First let me deal with MRI. In simple terms, multi-parametric MRI uses a combination of more powerful magnets and more sophisticated software to provide different enhanced views of the prostate. As such it is an improvement over both MRI and MRIS. The accuracy of multi-parametric MRI is therefore greater in many but not all circumstances. It remains the case that MRI in whatever guise still finds it difficult to differentiate BPH, prostatitis or prostate cancer. There has been and still is an ongoing debate among professionals as to the relative virtues and failings of MRI and TRUS. There is certainly no consensus nor is there anything that provides a guaranteed accurate answer. It could be useful, since it provides more data to actually use both. However, let me say that whatever flavour of MRI is used, it is seen by most medical professionals as a precursor to a biopsy, not an alternative to a biopsy. In fact, the multi-parametric MRI is commonly used for more accurate guided biopsy. That is considered one of its greatest strengths. It is also considered particularly accurate in diagnosing recurring cancers after treatment and establishing tumour volume, tumour grade or tumour stage after a biopsy. Although TRUS is also used largely for guided biopsy, the advanced prostate cancer risk assessment which I recommend, uses TRUS as one important component. In my APCRA the most important aspect is to use the data from a whole series of tests including TRUS and questionnaires in aggregate. It is the use of such data in aggregate that makes the assessment more accurate and although it cannot be legally called a diagnosis, it is in my view the best non-invasive alternative to biopsy. The important difference between TRUS and MRI is that MRI gives you a static image with a higher resolution than you get from TRUS. However, the advantage of TRUS is that it provides a dynamic image of what is happening inside the prostate including the blood flows. It is the pattern and strength of those blood flows in conjunction with the position of a possible lesion as shown by the TRUS that can give an experienced diagnostician an insight into whether one is dealing with BPH, prostatitis, or a genuine prostate cancer. I hope that helps you understand the differences in the technologies but the bottom line is that if you are looking for something that is a non-invasive alternative to a prostate biopsy, you would probably be best served with my APCRA.
