Drinking even "moderate" amounts of alcohol increases dementia risk, US research suggests.

The findings, presented at an international conference, challenge the notion that some alcohol could be good for ageing brains.

People who stick to recommended alcohol limits are still at risk, as well as bingers and heavy drinkers, according to the work.

The study tracked the health over 20 years of 1,300 women in their mid-60s.

Women who switched from abstinence to drinking over the course of the study also increased their risk.

Those who drank alcohol "in moderation", meaning seven to 14 alcoholic drinks a week, were also more likely to develop problems with memory and brain functioning that can be a warning sign of future dementia.

The lower end of this range falls within the UK's recommended limit for women, but since alcohol measures in the US are larger than in the UK, 14 drinks a week would exceed this UK weekly cut off.

And since the study only looked at women, it is not possible to say if the same link will apply in elderly men.

She told the Alzheimer's Association International Conference that it might be that brains become more vulnerable to the effects of alcohol as we age.

Some UK experts have recommended alcohol limits should be even tighter for older people for this reason.

At the same meeting, another group of US researchers presented more work showing the potential harms of binge drinking.

Among the 5,075 men and women they studied, those who reported heavy bouts of drinking - at least one episode per month - were more likely to experience dementia-like problems.

Fortnightly binges doubled the risk.

Drinking alcohol can cause your blood pressure and blood cholesterol to rise which, in turn, can damage the blood vessels supplying the brain, causing problems like vascular dementia.

Men are advised to drink no more than three to four units of alcohol a day, and women no more than two to three units a day. A unit of alcohol is equal to about half a pint of normal strength lager, a small glass of wine or a pub measure (25ml) of spirits.

Dr Marie Janson, of Alzheimer's Research UK ,said: "In a country with major concerns over binge drinking, these new findings should be taken seriously by people of all ages.

"There is mounting evidence linking alcohol consumption to cognitive decline, but this research delves deeper by examining the effects of different drinking patterns in more detail.

"These researchers found that in older people, even moderate drinking may have a harmful effect, in contrast to some previous research suggesting that moderate drinking may bring benefits.

"Such differing findings underline the need for more in-depth studies to tease out how different drinking patterns affect cognition.

"Many people will drink to relax and it's important to keep an eye on the amount of alcohol we consume."

She said that the best advice was to keep alcohol consumption light throughout life to reap some benefits and protect against the risks of over-indulging.

It's the conversion of alcohol into acetaldehyde which is bad for the brain. Don't really need it until you're going six feet under.

The three enzymes which can convert alcohol to acetaldehyde are:

· alcohol dehydrogenase (ADH) · cytochrome P450 (CYP2E1) · catalase

These three enzymes are each found in different parts of the body and each of them handles the hydrogen atoms which are stripped off from the alcohol molecule in a different way. Let us first look at alcohol dehydrogenase.

Alcohol dehydrogenase: The name "alcohol dehydrogenase" sounds like quite a mouthful, but it is quite self-explanatory if we break it down into its component parts. "de-" is a prefix which means "to remove". We find it in such words as "dethrone" which means "to remove from the throne". "-ase" is a suffix which means "enzyme". Any time you see a chemical term which ends in the suffix "-ase" you know that you are dealing with an enzyme. "hydrogen" means "hydrogen" of course. So "de-hydrogen-ase" means "an enzyme which removes hydrogen atoms", and "alcohol dehydrogenase" means "an enzyme which removes hydrogen atoms from the alcohol molecule". The name alcohol dehydrogenase is sometimes abbreviated to ADH.

Alcohol dehydrogenase is the workhorse of the alcohol enzymes--it breaks down the majority of the alcohol that enters the human body. Alcohol dehydrogenase is actually the name for a family of enzymes which break down alcohol--each of which has a slightly different molecular structure. Researchers have identified as many as 10 varieties of the alcohol dehydrogenase molecule. All of them bring about the same chemical reaction--the difference is that some varieties of alcohol dehydrogenase work more efficiently than others. As we shall see below, these variations in the alcohol dehydrogenase molecule can explain why some individuals react differently to alcohol than others.

The alcohol dehydrogenase molecules do their work primarily in the stomach and the liver, although traces of them are found in other tissues as well. The hydrogen which is released when alcohol dehydrogenase turns alcohol into acetaldehyde is bound to a compound called NAD+ (Nicotinamide Adenine Dinucleotide) to form NADH (this is short for Nicotinamide Adenine Dinucleotide plus Hydrogen). Cytochrome P450 2E1 (CYP2E1): In light social drinkers nearly all the alcohol consumed is taken care of by alcohol dehydrogenase as described above. However, the enzyme Cytochrome P450 2E1 (abbreviated CYP2E1) becomes quite active in metabolizing alcohol in chronic heavy drinkers. CYP2E1does its work in the liver. The hydrogen released by this reaction is bound to oxygen and to NADPH to form water and NADP+. This reaction takes energy rather than producing it.

Catalase: Catalase is found in tiny organs inside of cells called peroxisomes. Catalase is found all over the human body. When catalase turns alcohol into acetaldehyde the hydrogen which is released is bound to hydrogen peroxide molecules which then become water. Although catalase is active everywhere in the body, catalase is of particular interest to researchers because it metabolizes alcohol in the brain. The acetaldehyde released into the brain by the metabolism of alcohol by catalase has the potential to combine with neurotransmitters to form new compounds known as THIQs (tetrahydroisoquinolines, also sometimes called TIQs). Some researchers believe that THIQs are the cause of alcohol addiction and that the presence of THIQs distinguishes addicted drinkers from social drinkers. Other researches strongly dispute the validity of the THIQ hypothesis of alcohol addiction.. The actual role of THIQs remains controversial and a topic for further research.
