I would also add:
a. unless tort reform is implemented, medical costs will continue to rise because physicians will continue practicing defensive medicine.
b. unless Health Savings Accounts are more widely promoted and properly explained to the general public along with the benefits of a high deductible catastrophic plans, insurance premiums on plans that cover everything under the sun that few people will need or use will continue to increase.
c. EMTALA, the largest unfunded mandate on hospitals since Medicaid, needs to be repealed ASAP - illegals are disproportionately represented among Emergency Department patients covered by EMTALA - US citizen taxpayers are covering the costs of EMTALA directly thru tax subsidies and indirectly thru higher costs of hospital care for insured patients.

This, and all other unfunded mandates such as No Blockhead (or illegal) Left Behind, should either be repealed or just ignored by the states.
