Casualties of the conflict in Iraq since 2003 (beginning with the 2003 invasion of Iraq, and continuing with the ensuing occupation and insurgency) have come in many forms, and the accuracy of the information available on different types of Iraq War casualties varies greatly.

For troops in the U.S.-led multinational coalition, the death toll is carefully tracked and updated daily, and the names and photographs of those killed in action as well as in accidents have been published widely. A total of 4,486 U.S. soldiers were killed in Iraq between 2003 and 2012.[11] Regarding the Iraqis, however, information on both military and civilian casualties is both less precise and less consistent. Estimates of casualty levels are available from reporters on the scene, from officials of involved organizations, and from groups that summarize information on incidents reported in the news media.

The word "casualties" in its most general sense includes the injured as well as the dead. Accounts of the number of coalition wounded vary widely, partly because it is not obvious what should be counted: should only those injuries serious enough to put a soldier out of commission be included? Do illnesses or injuries caused by accidents count, or should the focus be restricted to wounds caused by hostile engagement? Sources using different definitions may arrive at very different numbers, and sometimes the precise definition is not clearly specified.

BAGHDAD – Al Qaeda-inspired militants pushed deeper into Iraq's Sunni heartland Wednesday, swiftly conquering Saddam Hussein's hometown of Tikrit as soldiers and security forces abandoned their posts and yielded ground once controlled by U.S. forces.

The advance into former insurgent strongholds that had largely been calm before the Americans withdrew less than three years ago is spreading fear that Prime Minister Nouri al-Maliki, struggling to hold onto power after indecisive elections, will be unable to stop the Islamic militants as they press closer to Baghdad.

Fighters from the Islamic State of Iraq and the Levant militant group took control Tuesday of much of Mosul, sending an estimated half a million fleeing from their homes. As in Tikrit, the Sunni militants were able to move in after police and military forces melted away after relatively brief clashes.

The group, which has seized wide swaths of territory, aims to create an Islamic emirate spanning both sides of the Iraq-Syria border.

The capture of Mosul -- along with the fall of Tikrit and ISIL's earlier seizure of the western city of Fallujah -- have undone hard-fought gains against insurgents in the years following the 2003 invasion by U.S.-led forces.

The White House said the security situation has deteriorated over the past 24 hours and that the United States was "deeply concerned" about ISIL's continued aggression.(2 images)

As fighters from the Islamic State of Iraq and Syria on Wednesday tightened their grip on Iraq’s second largest city, captured the hometown of Saddam Hussein and pushed to less than 100 miles from the Iraqi capital, the Senate Foreign Relations Committee met to question the man President Barack Obama has nominated to be the next ambassador to Iraq. Yet not a single senator asked him directly about the Iraqi government’s apparent loss of control.

They also didn’t ask questions of the man who sat next to him, the current U.S. ambassador to Iraq.

No one pressed for answers about how many U.S. weapons supplied to the Iraqi forces had ended up in insurgents’ hands as Iraqi forces shed their uniforms and fled their posts. Or what the fate will be of the U.S. military assistance program to Iraq, on which American taxpayers have already spent $14 billion. Or why they thought the American training program for Iraq’s military had resulted in such a humiliating rout.

Elsewhere on Capitol Hill, the House Armed Services Committee also showed a decided lack of interest in Iraq developments. At a daylong hearing, Secretary of Defense Chuck Hagel fielded not one question about Iraq. Admittedly, the hearing topic was the prisoner exchange that delivered five Taliban officials to Qatar from Guantanamo in return for captured Army Sgt. Bowe Bergdahl, but there was no limitation on what the committee members could ask.

A Senate staffer, asked about the omission, acknowledged surprise at the lack of curiosity from the Foreign Relations Committee about a country where nearly 4,500 Americans had died.

...In his opening statement, Stuart Jones, the current U.S. ambassador to Jordan who’s been nominated to go to Iraq, only referred once to events in Mosul, where fighters for the Islamic State of Iraq and Syria captured two airports, three military bases and freed thousands of suspected terrorists from prisons they captured.

“Monday’s coordinated attack on Mosul in which militants overran parts of the city highlights just how dangerous this group is,” Jones said in his opening statement. “We will continue to work with our international partners to meet the needs of those who have been displaced. And we will look for ways to support the government and the security forces.”

Jones assured senators that ensuring the safety of the 5,300 U.S. personnel who work at the U.S. Embassy in Baghdad and thousands of other U.S. citizens living in Iraq would be his “highest priority.”

But the senators at the one-hour hearing, which also featured the nominees to be ambassador to Egypt and Qatar, were far more interested in the turbulent political situation in Egypt and the memorandum of understanding between the U.S. and Qatar over the five released Taliban officials.
