Study Summary The goal of this retrospective study from the Mayo Clinic was to compare the risk for dialysis and mortality within 30 days after exposure to intravenous iso-osmolar iodinated contrast material (10,673 patients). These patients were compared with 10,673 patients who did not receive contrast. The risks for acute kidney injury (serum creatinine level > 0.5 mg/dL above baseline with 1-3 days of contrast material exposure), emergency dialysis, and short-term mortality did not significantly differ between the group that was exposed to contrast and the group that was not. Contrast material exposure was not an independent risk factor for short-term dialysis and mortality, even among patients at high risk for nephrotoxicity (acute or chronic renal insufficiency, diabetes mellitus, or congestive heart failure). Viewpoint The use of intravenous iodinated contrast material is common and is invaluable for imaging evaluation of various medical conditions, despite residual concern over potential nephrotoxicity.[1] In fact, practice guidelines have expressed caution about the use of contrast material among patients at high risk for renal failure.[2] A recent meta-analysis reported no significant difference in the incidence of acute renal injury between patients who received iso-osmolar agents and those who received low-osmolar agents.[3] The strength of this case/control study is that it assessed the potential extra risk for contrast-induced complications in large cohorts of baseline-riskmatched patients. The results and conclusions of this report suggest that the long-standing concern about the use of intravenous iodinated contrast material, even in patients with compromised renal function, may need to be reassessed and tempered.
