How do you figure that? That would only happen if the US surrenders, which I doubt would be the case.

Once nukes are flying, it's pretty much a matter of launching them all. Pretty sure that's what the US would do, so the Russians would need to launch theirs while they can.
mostly likely only a few coastal cities will actually get nuked.
