Mike Rivero: This photo confirms a likely cause of what happened. It is clear that the cracks which appeared in 2009 were not repaired, merely patched over. Water continued to leak through, washing out a void underneath, leaving the concrete spillway unsupported from below. When the void grew large enough, the spillway collapsed. (1 image)
