list 1 Qualify Use the Lifeline National Verifier to enter your information, create an account, and see if you qualify.
list 2 Choose a phone or internet company If you qualify, you will have 90 days to Choose a phone or internet company and sign up for service with them. You can also ask your current company to apply your Lifeline benefit to a service you are already getting.
list 3 Sign Up Start getting phone or internet service with your Lifeline benefit.
You may need to show other documents. If the Lifeline National Verifier is not able to prove that you qualify with only the information you give us, we may ask you to show official documents as proof.
If we ask you for official documents, you will be able to upload the documents right away or save it and come back later. See the full list of accepted documents.
You have 45 days after you start the application to finish it.

I checked to see whether it's legit. On sending it my secy said 'I just did it based on "substantial loss of income".... I went through the process and got an email back that says I qualify. Now I have to contact Charter and tell them' meaning the last step is notifying our Charter/Spectrum ISP. I've applied but not heard back yet.
