Throughout human history and even now in much of the world, most marriages are cousin marriages. The genetic risk is only 4% greater for first generation marriages than for non-related marriages. However, the risk increases for second generation marriages (their parents were also first cousins) and subsequent first cousin marriages. That is why in small closed societies with very little, if any, out-breading, recessive genetic disorders become common. That can be greatly eliminated by culling (killing) the children with genetic defects. In effect, that is what happened in prehistoric times where the death rate was so high that mostly only fittest survived.
